The Major Basic Instrument Set is primarily used in any surgical case that involves the more important, difficult, and hazardous procedures- major surgery. These are commonly extensive ones that involve organ removal or repair. Unlike minor surgery, a major one is life-threatening, for it involves operations within or upon the contents of the abdominal, pelvic, cranial, or thoracic cavities. Many factors – locality, condition of patient, case prognosis and operation sophistication – constitute a hazard to life or function of tissues, an organ or several organs. The recovery time can be lengthy and may involve a stay in the intensive care unit or several days in the hospital. A total of one hundred seventy-two (172) instruments compose the Major Basic Instrument Set. This set has most of the equipment seen in a standard minor basic instrument set. The major set relatively has more varieties of forceps and retractors. Other special instruments are also incorporated in the major set. There are three surgical knife handles used to hold blades indicated for creating several types of incisions. There are laparotomy rings designed to be used for gastroenterological and urological surgical procedures. The common scissors are included: Operating Scissors for general surgical procedures to cut sutures, gauze and other similar materials, Mayo Scissors for cutting heavy fascia and sutures, and Metzenbaum Scissors for cutting delicate tissues. The basic forceps – thumb, tissue, DeBakey, Adson, Skillman mosquito, Allis, Ochsner – are included too. There are additional haemostat forceps like the Meeker and Mixter forceps. There is also the Babcock, for holding a short length of intestine without compressing it. There are several Mayo Hegar needle holders too. Several retractors are also in the Major Basic Instrument Set: the Army Navy for large muscles and the Balfour for giving maximum access to tissues below. There are more instruments like different suction tubes and a tray.
